We require all new clients to complete the "New Client Form." You can download the form prior to your first visit by simply clicking the button below. 

 

This information is critical to keeping your Playful Pet safe, happy and healthy while you're at work or out-of-town.

Vaccination Policy

On or before your dog's first visit to Playful Pets, we ask that you provide proof of the following current vaccinations:

• Rabies

• DHPP - Distemper, Hepatitis, Parainfluenza, Parvo​

• Spay/neuter - (Dogs older than nine months)

• Flea/tick preventative

• Bordetella (Kennel Cough)
